Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a skilled setting
- Administer medications and treatments accurately, following care plans
- Monitor patient status and promptly report changes to the care team
- Document patient interactions, vitals and interventions accurately and timely
- Collaborate with nurses, CNAs and interdisciplinary staff to deliver coordinated care
- Support residents with activities of daily living while preserving dignity and independence
Qualifications
- Current LPN license in Pennsylvania or eligible for PA licensure
- Active basic life support (BLS) certification or willingness to obtain
- Demonstrated clinical experience as an LPN in long-term care, skilled nursing, or similar setting preferred
- Strong communication skills and a team-oriented attitude
- Ability to pass background checks and meet facility onboarding requirements
Education
- Graduate of an accredited practical nursing program or LPN diploma
Knowledge & Skills
- Medication administration and documentation best practices
- Basic wound care and urinary catheter care
- Familiarity with electronic health records or willingness to learn
- Ability to assess and report changes in patient condition
- Time management and prioritization in a busy clinical environment
- Compassionate bedside manner and resident-centered approach
