Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents in a skilled setting
Administer medications and treatments accurately, following care plans
Monitor patient status and promptly report changes to the care team
Document patient interactions, vitals and interventions accurately and timely
Collaborate with nurses, CNAs and interdisciplinary staff to deliver coordinated care
Support residents with activities of daily living while preserving dignity and independence

Qualifications

Current LPN license in Pennsylvania or eligible for PA licensure
Active basic life support (BLS) certification or willingness to obtain
Demonstrated clinical experience as an LPN in long-term care, skilled nursing, or similar setting preferred
Strong communication skills and a team-oriented attitude
Ability to pass background checks and meet facility onboarding requirements

Education

Graduate of an accredited practical nursing program or LPN diploma

Knowledge & Skills

Medication administration and documentation best practices
Basic wound care and urinary catheter care
Familiarity with electronic health records or willingness to learn
Ability to assess and report changes in patient condition
Time management and prioritization in a busy clinical environment
Compassionate bedside manner and resident-centered approach
