Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct care to residents, promoting dignity and comfort
Administer medications and treatments accurately and safely
Perform thorough assessments and communicate changes in condition to the care team
Document care clearly and timely in the resident chart and EHR
Collaborate with nurses, therapists, and family members to create person-centered plans
Assist with wound care, ADLs, and skilled nursing procedures as ordered
Participate in quality improvement initiatives and safety protocols

Qualifications

Current LPN license in Pennsylvania (active, in good standing)
Completion of an accredited practical nursing program
Basic Life Support (BLS) certification required; ACLS or other certs a plus
Clinical experience in a long-term care or skilled nursing setting preferred
Clear background check and up-to-date immunizations as required by facility
Strong communication skills and a team-first attitude

Education

Graduate of an accredited Licensed Practical Nurse (LPN) program
Current Pennsylvania LPN license
BLS (Basic Life Support) certification

Knowledge & Skills

Solid knowledge of medication administration and documentation
Comfortable performing resident assessments and vital signs monitoring
Familiarity with wound care, infection control, and safe patient handling
Experience using electronic health records (EHR) and charting systems
Good clinical judgment, prioritization, and time-management skills
Compassionate bedside manner and ability to work collaboratively
