Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in accordance with care plans
- Administer medications and treatments accurately and document according to facility policy
- Monitor resident vital signs and report changes to the care team for timely intervention
- Assist with activities of daily living while promoting independence and dignity
- Communicate clearly with residents, families, and interdisciplinary teams to coordinate care
- Support admissions, discharges, and transfers with thorough clinical handoffs
- Contribute to a safe, clean environment by following infection control and safety protocols
Qualifications
- Current LPN license (or equivalent) — Pennsylvania license preferred or eligibility to obtain
- Completion of an accredited practical/vocational nursing program
- Basic Life Support (BLS) / CPR certification or willingness to obtain
- Strong clinical assessment and medication administration skills
- Excellent communication skills and a team-oriented attitude
- Prior long-term care or skilled nursing experience is a plus
Education
- Graduation from an accredited LPN/Vocational Nursing program
- High school diploma or GED
Knowledge & Skills
- Skilled in medication administration and accurate documentation
- Comfortable with resident assessments and reporting changes in condition
- Knowledge of infection control practices and safety procedures
- Ability to prioritize tasks in a fast-paced clinical environment
- Compassionate bedside manner and strong interpersonal skills
