Roles & Responsibilities

• Provide compassionate direct patient care, focused on safety and dignity
• Administer medications and treatments according to policy and provider orders
• Monitor patient status and report changes promptly to the care team
• Document nursing assessments, interventions, and patient responses accurately
• Collaborate with nurses, providers, and support staff to deliver coordinated care
• Assist with ADLs and promote patient comfort and mobility
• Support infection control and facility safety practices at all times
• Contribute to a positive patient experience through clear communication and empathy

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania
• Current BLS/CPR certification required
• Previous LPN experience in a clinical setting preferred
• Ability to pass facility background and health screenings as required
• Strong communication skills and a team-oriented mindset
• Reliable, professional, and patient-focused approach to care

Education

• Graduation from an accredited practical nursing program
• High school diploma or GED

Knowledge & Skills

• Solid knowledge of nursing fundamentals and LPN scope of practice
• Medication administration and basic IV/route familiarity as applicable
• Patient assessment, vital signs, and documentation best practices
• Basic wound care and infection prevention techniques
• Electronic health record (EHR) charting experience preferred
• Time management, prioritization, and teamwork in fast-paced settings
