Roles & Responsibilities

Provide compassionate direct care to residents, supporting daily living needs
Administer medications and treatments safely and according to policy
Monitor, assess and document patient vitals and condition changes
Collaborate with nurses, providers and care team members to deliver coordinated care
Educate residents and families on care plans and promote resident dignity
Maintain infection control standards and a safe care environment
Participate in shift handoffs and accurate charting to ensure continuity of care

Qualifications

Active LPN license in good standing
Completion of an accredited practical/vocational nursing program
Current CPR/BLS certification (or willingness to obtain)
Ability to pass background check and required screenings
Strong communication skills and a team-oriented mindset
Previous long-term care or skilled nursing experience is a plus

Education

Diploma or certificate from an accredited LPN/Practical Nursing program

Knowledge & Skills

Medication administration and basic clinical assessments
Patient/resident documentation and charting accuracy
Basic wound care and infection control practices
Time management and prioritization in a fast-paced setting
Compassionate communication with residents, families, and staff
Familiarity with standard nursing procedures and safety protocols
