Roles & Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans in collaboration with the interdisciplinary team
• Provide expert pain and symptom management, IV therapy, and complex wound care
• Educate patients and caregivers on medication, treatment options, and discharge planning
• Coordinate care with physicians, providers, and vendors to ensure seamless patient support
• Delegate tasks to LPNs and supervise home health aides to maintain quality care
• Accurately document all care using McKesson and comply with state/federal regulations
• Advocate for patients' needs and facilitate safe discharges or transfers

Qualifications

• Active Vermont RN license (compact license accepted - apply upon acceptance)
• Minimum 2 years of nursing experience, preferably in hospice or home health
• Strong assessment, case management, and delegation skills
• Ability to work independently and as part of a collaborative team
• Commitment to providing compassionate, patient-centered care

Knowledge & Skills

• Expertise in hospice nursing, pain management, and symptom control
• Proficient in IV therapy, wound VAC, and complex wound care
• Skilled in patient assessment, care planning, and documentation
• Knowledge of infection control standards and evidence-based practices
• Experience with McKesson or similar EMR systems
• Ability to perform peer reviews and educational presentations
