Roles & Responsibilities

• Evaluate and treat residents with speech, language, cognitive-communication and swallowing disorders in the SNF setting
• Develop individualized treatment plans and document progress according to facility and Medicare guidelines
• Collaborate with nursing, therapy colleagues, and families to support resident-centered goals
• Provide clear, timely documentation and discharge recommendations
• Educate staff and caregivers on strategies to support carryover and safety
• Adapt interventions to meet the needs of an older adult population in a skilled nursing environment

Qualifications

• Active state SLP license (required)
• 1–3 years of clinical experience in skilled nursing or related settings preferred
• Comfortable working PRN shifts and adjusting to facility-specific workflows
• Strong documentation skills and familiarity with SNF/long-term care processes
• Current CPR certification (or willingness to obtain) and background clearances as required by facility

Education

• Master’s degree in Speech-Language Pathology or equivalent (typical requirement for SLP roles)

Knowledge & Skills

• Skilled in dysphagia management, cognitive-communication therapy, and language rehabilitation
• Good clinical reasoning and individualized treatment planning
• Excellent interpersonal and communication skills for working with residents and interdisciplinary teams
• Proficient documentation practices and understanding of SNF regulatory expectations
• Ability to manage a varied caseload and prioritize tasks in a busy facility
