Roles & Responsibilities

• Assess speech, language, cognition, and swallowing function using evidence-based tools
• Develop individualized treatment plans that address patients' goals and safety
• Implement skilled SLP interventions in a Skilled Nursing Facility setting
• Collaborate with interdisciplinary team members to coordinate care and optimize outcomes
• Document evaluations, progress notes, and discharge plans accurately and timely
• Provide patient and family education to support carryover and safe swallowing strategies
• Participate in quality initiatives and contribute to a positive, patient-centered culture

Qualifications

• ASHA CCC-SLP required
• Active or eligible for Ohio state SLP license
• 1-3 years of clinical experience, SNF experience preferred
• Comfortable with skilled documentation and working in a fast-paced rehab environment
• Ability to pass required background checks and facility onboarding

Knowledge & Skills

• Skilled in speech-language pathology evaluations and treatment plans
• Experience with dysphagia assessment and safe swallowing strategies
• Strong clinical documentation and electronic medical record skills
• Ability to collaborate effectively with nursing, therapy, and ancillary staff
• Patient-centered communication and family education skills
