Roles & Responsibilities

• Evaluate patients' speech, language, cognition and swallowing function using evidence-based assessments
• Develop individualized treatment plans that set clear, measurable goals and drive meaningful outcomes
• Deliver skilled speech-language therapy interventions in a skilled nursing facility setting
• Document progress, discharge planning and skilled justification thoroughly and on time
• Collaborate with nurses, PT/OT and interdisciplinary team members to coordinate patient-centered care
• Educate patients, families and caregivers on strategies to support communication and safe eating
• Maintain professional standards, participate in team meetings, and support quality improvement initiatives

Qualifications

• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) — as noted in the posting
• Active SLP license in New Hampshire or eligibility to obtain one prior to start
• Comfort working in a Skilled Nursing Facility (SNF) environment
• Any experience level welcome — new grads encouraged to apply
• Valid immunizations and ability to pass background checks as required by facility
• Strong clinical reasoning, documentation skills, and professional communication

Education

• Master’s degree in Speech-Language Pathology preferred; clinical training and licensure required (ASHA CCC-SLP preferred or in process)

Knowledge & Skills

• Skilled in dysphagia evaluation and management and safe swallowing strategies
• Experience with communication and cognitive-communication disorders common in SNF populations
• Proficient documentation and familiarity with skilled therapy justification
• Ability to collaborate effectively with nursing and rehab teams
• Patient-centered approach with strong teaching and caregiver training skills
