Roles & Responsibilities
- Evaluate patients' speech, language, cognition and swallowing function using evidence-based assessments
- Develop individualized treatment plans that set clear, measurable goals and drive meaningful outcomes
- Deliver skilled speech-language therapy interventions in a skilled nursing facility setting
- Document progress, discharge planning and skilled justification thoroughly and on time
- Collaborate with nurses, PT/OT and interdisciplinary team members to coordinate patient-centered care
- Educate patients, families and caregivers on strategies to support communication and safe eating
- Maintain professional standards, participate in team meetings, and support quality improvement initiatives
Qualifications
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) — as noted in the posting
- Active SLP license in New Hampshire or eligibility to obtain one prior to start
- Comfort working in a Skilled Nursing Facility (SNF) environment
- Any experience level welcome — new grads encouraged to apply
- Valid immunizations and ability to pass background checks as required by facility
- Strong clinical reasoning, documentation skills, and professional communication
Education
- Master’s degree in Speech-Language Pathology preferred; clinical training and licensure required (ASHA CCC-SLP preferred or in process)
Knowledge & Skills
- Skilled in dysphagia evaluation and management and safe swallowing strategies
- Experience with communication and cognitive-communication disorders common in SNF populations
- Proficient documentation and familiarity with skilled therapy justification
- Ability to collaborate effectively with nursing and rehab teams
- Patient-centered approach with strong teaching and caregiver training skills
