Roles & Responsibilities
- Provide skilled speech-language pathology evaluations and treatment for adults in a home health / SNF setting
- Develop individualized care plans and document progress clearly and professionally
- Collaborate with interdisciplinary team members, family, and caregivers to support patient goals
- Deliver evidence-based therapy for swallowing, communication, and cognitive-communication disorders
- Educate patients and families on strategies to improve safety and functional communication
- Track outcomes, complete timely documentation, and ensure regulatory compliance
Qualifications
- Active SLP license in Missouri (or compact/state equivalent) required
- Certificate of Clinical Competence (CCC-SLP) preferred
- 1-3 years of clinical experience in SLP, with home health or SNF experience a strong plus
- Current CPR/BLS per facility requirements
- Reliable transportation and ability to travel to patient homes as required
- Strong communication skills and a patient-centered approach
Education
- Master's degree in Speech-Language Pathology (typical requirement for clinical practice)
- ASHA CCC-SLP preferred
Knowledge & Skills
- Skilled in dysphagia assessment and management
- Experience with neurogenic and geriatric communication disorders
- Familiarity with documentation standards for home health and SNF settings
- Ability to create practical, functional treatment plans tailored to daily living
- Comfortable collaborating with nursing and therapy teams
