Roles & Responsibilities
- Evaluate residents and perform skilled speech-language pathology assessments with precision
- Develop individualized treatment plans focused on functional outcomes and quality of life
- Provide evidence-based therapy for communication, cognition, and swallowing disorders
- Collaborate with nursing, therapy colleagues, and families to coordinate holistic care
- Document progress and treatment accurately in the patient chart and meet facility timelines
- Educate caregivers and staff on strategies to support carryover and safety
- Participate in interdisciplinary team meetings to advocate for resident needs
Qualifications
- Current state license to practice as a Speech-Language Pathologist in Tennessee
- CCC-SLP from ASHA preferred or working toward certification
- 1-3 years of clinical experience, SNF or adult rehab experience a plus
- Strong clinical skills in dysphagia management, cognitive-communication, and functional therapy
- Reliable transportation, ability to pass background checks and facility onboarding requirements
- Excellent communication and teamwork skills
Education
- Master's degree in Speech-Language Pathology or equivalent
- ASHA Certificate of Clinical Competence (CCC-SLP) preferred
Knowledge & Skills
- Proficiency in dysphagia assessment and treatment protocols
- Experience with SNF therapy workflows and Medicare documentation
- Skilled at creating practical, functional treatment plans
- Comfortable with electronic medical records and timely documentation
- Ability to train and coach caregivers and interdisciplinary staff
- Strong clinical reasoning and patient-centered problem solving
