Roles & Responsibilities

• Evaluate residents and perform skilled speech-language pathology assessments with precision
• Develop individualized treatment plans focused on functional outcomes and quality of life
• Provide evidence-based therapy for communication, cognition, and swallowing disorders
• Collaborate with nursing, therapy colleagues, and families to coordinate holistic care
• Document progress and treatment accurately in the patient chart and meet facility timelines
• Educate caregivers and staff on strategies to support carryover and safety
• Participate in interdisciplinary team meetings to advocate for resident needs

Qualifications

• Current state license to practice as a Speech-Language Pathologist in Tennessee
• CCC-SLP from ASHA preferred or working toward certification
• 1-3 years of clinical experience, SNF or adult rehab experience a plus
• Strong clinical skills in dysphagia management, cognitive-communication, and functional therapy
• Reliable transportation, ability to pass background checks and facility onboarding requirements
• Excellent communication and teamwork skills

Education

• Master's degree in Speech-Language Pathology or equivalent
• ASHA Certificate of Clinical Competence (CCC-SLP) preferred

Knowledge & Skills

• Proficiency in dysphagia assessment and treatment protocols
• Experience with SNF therapy workflows and Medicare documentation
• Skilled at creating practical, functional treatment plans
• Comfortable with electronic medical records and timely documentation
• Ability to train and coach caregivers and interdisciplinary staff
• Strong clinical reasoning and patient-centered problem solving
