Roles & Responsibilities
- Assess speech, language, voice and swallowing function for skilled nursing residents
- Develop individualized treatment plans and measurable goals that improve quality of life
- Deliver skilled therapy services using evidence-based approaches and restorative techniques
- Document timely, accurate clinical notes and productivity per CRG policies
- Collaborate with nursing, rehab team members and families to coordinate care
- Provide caregiver and family education to support carryover of therapy strategies
- Maintain required productivity (90%) while ensuring resident-centered care
- Float between San Juan Care Center and Farmington Wellness & Rehab as needed
Qualifications
- State of New Mexico SLP license (or eligible to obtain) — required
- ASHA CCC-SLP preferred but not always required
- 1–3 years of clinical experience in a skilled nursing facility or related setting
- Current BLS/CPR certification
- Reliable transportation and flexibility to float between two facilities
- Ability to follow CRG policies and wear the required black scrubs
Education
- Master's degree in Speech-Language Pathology (or equivalent graduate degree)
Knowledge & Skills
- Skilled in dysphagia management and safe swallowing strategies
- Experience with geriatric communication and cognitive-communication disorders
- Strong documentation skills and familiarity with productivity expectations in SNF settings
- Excellent interpersonal skills for team-based care and family education
- Time management and caseload prioritization while floating between sites
- Ability to adapt treatment plans based on resident progress and facility needs
