Roles & Responsibilities

• Assess speech, language, voice and swallowing function for skilled nursing residents
• Develop individualized treatment plans and measurable goals that improve quality of life
• Deliver skilled therapy services using evidence-based approaches and restorative techniques
• Document timely, accurate clinical notes and productivity per CRG policies
• Collaborate with nursing, rehab team members and families to coordinate care
• Provide caregiver and family education to support carryover of therapy strategies
• Maintain required productivity (90%) while ensuring resident-centered care
• Float between San Juan Care Center and Farmington Wellness & Rehab as needed

Qualifications

• State of New Mexico SLP license (or eligible to obtain) — required
• ASHA CCC-SLP preferred but not always required
• 1–3 years of clinical experience in a skilled nursing facility or related setting
• Current BLS/CPR certification
• Reliable transportation and flexibility to float between two facilities
• Ability to follow CRG policies and wear the required black scrubs

Education

• Master's degree in Speech-Language Pathology (or equivalent graduate degree)

Knowledge & Skills

• Skilled in dysphagia management and safe swallowing strategies
• Experience with geriatric communication and cognitive-communication disorders
• Strong documentation skills and familiarity with productivity expectations in SNF settings
• Excellent interpersonal skills for team-based care and family education
• Time management and caseload prioritization while floating between sites
• Ability to adapt treatment plans based on resident progress and facility needs
