Roles & Responsibilities

• Provide compassionate bedside care for residents as a licensed practical nurse
• Administer medications and treatments accurately and safely
• Assess patients, monitor vital signs and report changes to the care team
• Document care in the medical record clearly and promptly
• Collaborate with nurses, therapists and physicians to deliver coordinated care
• Support admissions, discharges and care transitions with professionalism
• Promote infection control and safety practices across the unit

Qualifications

• Active LPN license (able to practice in New Hampshire as required)
• Previous LPN experience in long-term care or skilled nursing preferred
• Willingness to work day shifts and every-other-weekend coverage
• Current Basic Life Support (BLS) certification, or willingness to obtain
• Able to pass background check and meet facility onboarding requirements

Education

• Completion of an accredited LPN/LVN program (diploma or certificate)
• Active LPN license

Knowledge & Skills

• Strong clinical assessment and medication administration skills
• Comfortable with electronic medical records and documentation
• Good communication and teamwork with multidisciplinary staff
• Time management and prioritization in a fast-paced clinical setting
• Compassionate patient-centered care with an eye for detail
