Roles & Responsibilities
- Coordinate seamless care transitions for patients across the healthcare continuum
- Collaborate with interdisciplinary teams to develop comprehensive discharge plans
- Utilize Midas and Meditech systems to track patient progress and outcomes
- Advocate for patients while ensuring appropriate resource utilization
- Partner with social workers and clinical staff to support complex discharge needs
Qualifications
- Active RN license in Florida or compact state
- Minimum 1 year of case management experience
- Proficiency with Midas and Meditech electronic systems
- Strong communication and organizational skills
Knowledge & Skills
- Expertise in discharge planning and care coordination
- Familiarity with Midas and Meditech platforms
- Understanding of complex patient needs and resource navigation
- Ability to work collaboratively within a multidisciplinary team
