Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 4:00 PM Days
Hours per Week: 36
Candidate Pay Rate: $74 per hour

Roles & Responsibilities
- Develop personalized care plans in collaboration with physicians, patients, and families
- Coordinate patient care through interdisciplinary communication and case conferences
- Provide ongoing assessment of clinical, functional, and emotional status
- Teach patients and caregivers essential self-care techniques and medication management
- Document evaluations, treatment goals, and progress toward desired outcomes
Qualifications
- Active Registered Nurse license in Vermont and New Hampshire or unencumbered compact license
- Graduate of an accredited school of nursing
- Minimum 2 years of experience in Home Health
- Current BLS/BCLS certification
Education
- Graduate of an accredited school of nursing
Knowledge & Skills
- Patient assessment and care planning expertise
- Strong case management abilities
- Excellent patient education and communication skills
- Ability to work independently while collaborating with an interdisciplinary team
