Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with every other weekend rotation

Hours per Week: 40

Candidate Pay Rate: $82 per hour

Roles & Responsibilities

Coordinate care across all hospital areas to ensure seamless patient experiences
Complete initial assessments and utilization reviews using Interqual criteria
Develop comprehensive discharge plans that support patients and families
Collaborate with interdisciplinary teams to optimize patient outcomes
Navigate payer requirements while advocating for patients in need
Support individuals facing complex challenges including homelessness and lack of insurance

Qualifications

Active California RN license required
BLS certification required
Prior recent experience with Dignity Health facilities strongly preferred
Experience with Interqual and Cerner systems preferred
Strong communication and teamwork skills essential
Ability to adapt and reprioritize throughout the day

Knowledge & Skills

Case Management and Care Coordination expertise
Utilization Review and Discharge Planning proficiency
Interqual criteria application
Cerner electronic medical record system
Collaboration across multidisciplinary teams
Effective communication with patients, families, and payers
