Candidate Pay Rate: $49 per hour

Roles & Responsibilities

Deliver compassionate physical therapy care to patients across Sacramento County in their homes
Treat a diverse caseload including cardio, pulmonary, orthopedic, neurological, and geriatric patients
Help patients regain strength, mobility, and independence with personalized treatment plans
Collaborate with interdisciplinary teams to ensure the best outcomes for every patient
Document visits accurately and maintain compliance with all regulatory standards

Qualifications

Active Physical Therapy Assistant license in California
Prior home health experience preferred
New grads considered only in exceptional circumstances
Strong communication and time-management skills
Reliable transportation and willingness to travel within the greater Sacramento area

Knowledge & Skills

Home health care protocols
Patient assessment and treatment planning
Orthopedic, neurological, and cardiopulmonary rehabilitation
Documentation and compliance standards
Building rapport with patients and families
