Roles & Responsibilities

• Provide one-on-one outpatient physical therapy sessions focused on rehabilitation and patient-centered care
• Perform thorough assessments and create individualized treatment plans that promote recovery and function
• Document patient progress accurately and use the built-in documentation hour to keep records current
• Collaborate with a multidisciplinary rehab team to coordinate care and optimize outcomes
• Educate patients and families on home programs, preventative strategies, and functional progression
• Use clinic equipment and modalities safely and effectively to support treatment goals
• Contribute to a positive team culture by mentoring newer clinicians and sharing best practices

Qualifications

• Outpatient Physical Therapy experience (preferred)
• Familiarity with Epic electronic health record system (preferred)
• Comfortable and experienced in a one-on-one treatment model
• Strong documentation skills and attention to detail
• Excellent teamwork and communication skills in a collaborative clinic environment
