Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Conduct comprehensive Oasis Assessments for new patients
Manage Start of Care evaluations across four counties
Provide skilled physical therapy to elderly patients recovering from falls and fractures
Complete 4-6 meaningful patient visits daily using the point system
Document visits and track time using tablet technology
Build lasting relationships with patients in rural communities

Qualifications

Current Physical Therapist license in Indiana
Experience with Oasis Assessments required
Comfortable with Start of Care evaluations
Ability to work independently across multiple counties
Valid driver's license for travel between locations

Time management for 4-6 daily visits
