Contract Type: Travel Contract

Shift Type: Monday - Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide skilled physical therapy services to home health patients in the Valley City area
Evaluate and treat patients recovering from surgeries, falls, and other conditions
Develop and implement individualized care plans in collaboration with the healthcare team
Travel within a 60-mile radius to deliver care in patients' homes
Document patient progress using HomeCare/HomeBase EMR system
Educate patients and families on therapeutic exercises and safety techniques

Qualifications

Active North Dakota Physical Therapy License (required)
Current BLS certification (required)
Valid Driver's License and Auto Insurance (required)
Minimum 1 year of physical therapy experience
Previous home health experience strongly preferred
No prior travel experience needed - we welcome first-time travelers!

Knowledge & Skills

Solid clinical skills in physical therapy evaluation and treatment
Comfort working independently in a home health setting
Familiarity with HomeCare/HomeBase or similar EMR systems
Excellent communication and patient education abilities
Time management skills to effectively manage 5-7 patient visits per day
