Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Perform start of care, recertifications, and resumption of care for home health patients
Manage an average of 6.5 points per day, including regular visits and admissions
Conduct safety evaluations and occasional hospice teaching visits
Collaborate with clinical team and coordinate care plans
Document patient progress and ensure compliance with OASIS and regulatory standards

Qualifications

Current ND Physical Therapy License required
BLS certification required
Valid Driver's License and Auto Insurance required
Prefer home health experience and OASIS training

Knowledge & Skills

Skilled in home health therapy and OASIS documentation
Ability to perform comprehensive assessments and develop care plans
Strong time management and scheduling abilities
Effective communication and collaboration with interdisciplinary team
