Contract Type: Travel Contract

Shift Type: Monday-Friday with rotating weekends and holidays, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Conduct comprehensive patient evaluations and craft personalized treatment plans
Perform start-of-care visits, re-certifications, and discharges with confidence
Deliver skilled physical therapy services in patients' homes, making a real impact
Educate patients and families on therapeutic exercises and home safety
Collaborate with clinical managers and educators for seamless support
Maintain accurate documentation to track progress and outcomes

Qualifications

Active Physical Therapist license in Kentucky (required)
BLS certification from the American Heart Association
Experience in home health is preferred, but new grads are welcome to apply
Ability to manage a diverse caseload and adapt to various patient needs

Knowledge & Skills

Strong clinical knowledge in orthopedics, CHF, and pneumonia management
Familiarity with home health regulations and OASIS documentation
Excellent communication and problem-solving abilities
Comfortable using a tablet and phone for charting and communication
