Roles & Responsibilities

Deliver compassionate physical therapy services across four rural counties
Conduct comprehensive Oasis Assessments and Start of Care evaluations
Help elderly patients recover from falls and fractures with personalized treatment plans
Build meaningful connections with patients in their home environments
Track visits and time using modern tablet technology
Collaborate with a dedicated healthcare team serving Noble, Lagrange, Steuben, and DeKalb counties

Qualifications

Current Physical Therapist license in Indiana
Experience with Oasis Assessments and Start of Care documentation
Comfortable working independently in rural settings
Strong assessment and clinical reasoning skills
