Roles & Responsibilities

Deliver personalized physical therapy services directly in patients' homes throughout Yolo County
Assess and evaluate each patient's therapeutic needs while developing comprehensive care plans
Guide patients and families through rehabilitation exercises and therapeutic appliance use
Monitor progress and communicate changes to physicians while maintaining detailed clinical documentation
Collaborate with home health aides and coordinate care with interdisciplinary teams
Educate caregivers on therapy goals and techniques to support continued progress between visits

Qualifications

Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong oral and written communication skills with excellent organization abilities
Valid Physical Therapy license for California practice
Ability to work independently while thriving in a collaborative healthcare environment

Knowledge & Skills

Home health physical therapy assessment and treatment techniques
Patient evaluation including muscle strength, mobility, gait, and range of motion
Care plan development and goal-setting with patients and families
Documentation and clinical reporting requirements
Medicare and Medi-Cal compliance standards
Prosthetic device training and home safety evaluation
