Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes across Yolo County
Assess and evaluate each patient's strength, mobility, gait, and rehab potential
Create and update individualized care plans that help patients regain independence
Educate patients and families on therapeutic exercises and equipment use
Collaborate with physicians and care teams to track progress and adjust treatments
Support home health aides and caregivers as part of the rehab team
Document clinical notes and progress summaries that guide ongoing care
Identify additional home health needs and connect patients with the right resources

Qualifications

Active Physical Therapist license in California
Minimum 2 years of experience in acute, home care, or rehab settings
Current BLS certification
Strong communication and organizational skills
Comfortable working independently in home settings

Knowledge & Skills

Home health physical therapy techniques
Patient assessment and functional evaluation
Rehabilitation planning and goal setting
Caregiver education and training
Medicare and Medi-Cal compliance
Clear documentation and progress reporting
