Physical Therapist - Home Health
Location: Red Bluff, California

Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Deliver personalized physical therapy care to patients in their homes across Tehama County
- Perform Start of Care visits especially for orthopedic patients when nursing is not ordered
- Help patients recover from joint replacements and manage chronic conditions like CHF and COPD
- Complete approximately five meaningful visits each day while building lasting patient relationships
- Document care using HomeCare HomeBase EMR on your provided tablet
Qualifications
- Current Physical Therapist license in California
- BLS certification required
- Home Health experience preferred but will consider candidates with less than two years experience who have Home Health background
- Strong communication skills and passion for patient-centered care
Knowledge & Skills
- Physical Therapy expertise in home health settings
- Comfortable working independently in patient homes
- Familiarity with orthopedic and geriatric patient populations
- Proficient with EMR documentation and tablet technology
