Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $56 per hour

Roles & Responsibilities
- Provide compassionate physical therapy services to patients in their homes
- Conduct start-of-care assessments, recertifications, and resumption-of-care visits
- Deliver 5 to 6 meaningful patient visits each day, adjusting for admissions or travel
- Collaborate with a supportive clinical team available for guidance in the field
- Complete OASIS documentation and ensure high-quality care coordination
- Perform occasional safety evaluations or teaching visits in hospice situations
Qualifications
- Current ND Physical Therapy License
- BLS certification
- Valid driver's license and auto insurance
- Home Health experience and OASIS training preferred
- First-time travelers welcome
