Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes
Conduct start of care assessments, recertifications, and resumption of care visits
Deliver 5-6 meaningful patient visits per day helping individuals regain mobility and independence
Collaborate with clinical coordinators and fellow therapists for seamless patient care
Complete OASIS documentation to ensure quality patient outcomes
Perform occasional safety evaluations for hospice patients when needed

Qualifications

Current North Dakota Physical Therapy License required
BLS certification required
Valid driver's license and auto insurance required
Home Health experience and OASIS training preferred
First time travelers welcome to apply

OASIS documentation proficiency
Strong communication and patient education skills
Ability to work independently while staying connected with the care team
