Location: Santa Maria, CA

Start Date: 06-12-2026

End Date: 09-11-2026

Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with flexibility; every 3rd weekend required

Hours per Week: 40

Candidate Pay Rate: $57 per hour

Roles & Responsibilities

Provide occupational therapy services to patients in their homes, helping them regain independence and improve quality of life
Develop and implement individualized treatment plans based on patient assessments and goals
Collaborate with interdisciplinary team including PT, SLP, and nursing staff
Document patient progress in EMR system (Cerner) and complete Oasis assessments as required
Educate patients and families on home exercise programs and adaptive strategies

Qualifications

Current California Occupational Therapy license
BLS certification required
Minimum 1 year of experience as an Occupational Therapist
Experience with Oasis documentation is required
Home health experience preferred but not mandatory
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Proficient in EMR systems, especially Cerner
Strong clinical evaluation and treatment planning skills
Excellent communication and interpersonal skills for patient and family education
Ability to work independently and manage a geographically assigned caseload
Knowledge of Oasis assessment and CMS regulations
