Roles & Responsibilities

Provide skilled, compassionate nursing care to residents, ensuring safety and dignity
Administer medications and treatments accurately, following physician orders
Assess patient condition and report changes promptly to the care team
Document care and maintain clear, timely clinical records
Collaborate with CNAs, RNs, and interdisciplinary team members to create effective care plans
Educate residents and families on care routines and health management

Qualifications

Active LPN license in Pennsylvania (required)
Completion of an accredited LPN program
Current BLS/CPR certification (preferred or obtained upon hire)
Demonstrated ability to provide compassionate, patient-centered care
Strong communication skills and ability to work in a team environment

Education

Graduation from an accredited LPN program; High school diploma or equivalent

Knowledge & Skills

Medication administration and documentation best practices
Resident assessment and basic clinical monitoring
Care planning and implementation in long-term care settings
Infection control and safety procedures
Time management and prioritization in a busy clinical environment
