Roles & Responsibilities

Provide compassionate, hands-on patient care consistent with LPN scope of practice
Administer medications and treatments safely and document accurately
Monitor and record vital signs, reporting changes in patient condition promptly
Collaborate with RNs, physicians and the care team to support care plans
Maintain clear, timely charting in the patient record and communicate changes
Support patient comfort, education and discharge preparation with empathy

Qualifications

Valid LPN license (preferably active in Pennsylvania or eligible to practice in PA)
Current BLS/CPR certification (or willingness to obtain promptly)
Clinical experience in long-term care, skilled nursing, or similar settings preferred
Strong communication skills and a patient-centered approach
Ability to pass facility onboarding and background checks

Education

Licensed Practical Nurse (LPN) diploma or equivalent from an accredited program

Knowledge & Skills

Medication administration and documentation best practices
Basic assessment skills: vital signs, wound observation, ADL support
Familiarity with electronic medical records and charting
Infection control and safety procedures
Teamwork, time management, and clear clinical communication
