Roles & Responsibilities
- Provide compassionate, resident-focused nursing care that makes a real difference
- Administer medications and treatments safely and document accurately
- Monitor and record vital signs, recognizing and reporting changes in condition
- Collaborate with nurses, therapists, and care staff to deliver coordinated care
- Maintain clear, timely clinical documentation in the medical record
- Support residents with activities of daily living while promoting dignity and independence
- Follow infection control, safety protocols, and facility policies to ensure a safe environment
Qualifications
- Active LPN license in Pennsylvania (required)
- Completion of an accredited practical nursing program
- Current BLS (Basic Life Support) certification preferred
- Previous experience in long-term care or skilled nursing is a plus
- Strong communication skills and the ability to work well in a team
Education
- High school diploma or equivalent
- Graduation from an accredited practical nursing (LPN) program
Knowledge & Skills
- Medication administration and accurate charting
- Basic clinical assessments and vital sign interpretation
- Infection control and safety procedures
- Resident-centered care principles and ADL support
- Good documentation practices and electronic health record familiarity
- Effective teamwork and clear communication with clinical staff
