Roles & Responsibilities

Provide compassionate direct nursing care to residents, focusing on safety and dignity
Administer medications and treatments accurately and document thoroughly
Perform timely patient assessments and report changes to the care team
Collaborate with RNs, physicians, and interdisciplinary staff to coordinate care
Maintain accurate and professional clinical documentation in the health record
Assist with wound care, IV care as applicable, and other clinical procedures per facility policy
Educate residents and families on care plans and health maintenance in a friendly, approachable way

Qualifications

Current LPN licensure in Pennsylvania (required)
BLS/CPR certification (required or obtain upon hire)
Clinical experience in long-term care, skilled nursing, or similar setting preferred
Strong communication skills and the ability to work as part of a collaborative care team
Able to pass background checks and meet facility onboarding requirements

Education

Graduate of an accredited Practical Nursing (LPN/LVN) program
Current LPN license in Pennsylvania
BLS/CPR certification

Knowledge & Skills

Medication administration and documentation best practices
Basic clinical assessment and vital signs monitoring
Familiarity with infection control standards and safety protocols
Comfortable using electronic medical records (EMR) for charting
Effective interpersonal skills for patient- and family-centered care
