Roles & Responsibilities
- Provide compassionate direct patient care and bedside support
- Administer medications and treatments safely and accurately
- Assess patient conditions and report changes promptly to the care team
- Document care, observations, and communications in the medical record
- Collaborate with nurses, therapists, and providers to develop care plans
- Support patients and families with clear, respectful communication
- Assist with admissions, transfers, and safe patient handoffs
Qualifications
- Active Licensed Practical Nurse (LPN) license — Pennsylvania license required or eligible
- Current BLS/CPR certification (required)
- Comfortable working in a fast-paced long-term care or skilled nursing setting
- Strong communication skills and ability to work well within a team
- Ability to pass background check and drug screening as required
Education
- Completion of an accredited LPN/Vocational Nursing program
Knowledge & Skills
- Medication administration and documentation best practices
- Patient assessment, vital signs, and basic wound care
- Infection control and safe patient handling techniques
- Electronic medical record (EMR) charting and documentation
- Time management, prioritization, and effective clinical judgment
