Roles & Responsibilities
- Provide compassionate bedside nursing care to residents, ensuring dignity and respect
- Administer medications and treatments accurately and document in the medical record
- Monitor residents vital signs and report changes in condition promptly
- Collaborate with RNs, support staff and interdisciplinary team to implement care plans
- Support activities of daily living and assist residents with mobility and hygiene needs
- Communicate effectively with residents and families to provide updates and education
- Participate in quality improvement initiatives and maintain regulatory compliance
Qualifications
- Active LPN license and eligibility to practice in Pennsylvania
- Completion of an accredited practical nursing program
- Current BLS (Basic Life Support) certification or willingness to obtain
- Comfortable with medication administration, basic wound care and resident assessments
- Strong communication skills and ability to work as part of a multidisciplinary team
- Prior experience in long-term care or skilled nursing preferred
Education
- Graduation from an accredited practical nursing program (LPN diploma or certificate)
Knowledge & Skills
- Medication administration and documentation
- Basic wound care and treatment monitoring
- Vital signs assessment and basic clinical observations
- Electronic medical record/charting familiarity and accuracy
- Patient-centered communication and family engagement
- Time management, prioritization and teamwork in a busy clinical setting
