Contract Type: Travel Contract
Shift Type: Rotating 2-week cycle with required weekends. Week 1: Wednesday 1:00 PM - 10:30 PM; Thursday 9:00 PM - 6:30 AM (NOC); Saturday & Sunday 1:00 PM - 10:30 PM. Week 2: Wednesday 2:00 PM - 10:30 PM; Friday 2:00 PM - 10:30 PM; Saturday & Sunday 6:00 AM - 10:30 PM. (Mix of PM and night shifts — rotating schedule)
Hours per Week: 34
Candidate Pay Rate: $49 per hour

Roles & Responsibilities
- Communicate timely and clearly with physicians, families, and the care team about significant changes in resident condition
- Complete thorough resident assessments and maintain accurate, detailed charting and documentation
- Administer medications safely following the 5 Rights and monitor residents for medication effects
- Perform wound care and ongoing assessments to support resident healing and comfort
- Provide leadership on the floor by assessing situations, directing care, and assisting CNAs as needed
- Report and document any allegations of abuse, neglect, or misappropriation to nursing leadership promptly
- Maintain accurate resident records and ensure compliance with facility policies and regulations
- Pitch in with other duties as requested to ensure residents receive exceptional, compassionate care
Qualifications
- Current Wisconsin Registered Nurse (RN) license — required
- Current CPR/BLS certification — required
- Associate degree in nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred
- 1+ years of related long-term care or nursing experience preferred
- Strong attention to detail and excellent documentation skills
- Ability to communicate effectively with physicians, families, and interdisciplinary teams
- Comfort leading a floor and supporting CNAs in day-to-day resident care
- Must be able to follow abuse/neglect reporting requirements and facility protocols
Education
- Associate Degree in Nursing (required)
- Bachelor of Science in Nursing (BSN) preferred
Knowledge & Skills
- Medication administration and patient monitoring
- Wound care techniques and assessment
- Resident assessments and care planning
- Charting and clinical documentation best practices
- CPR/BLS and basic emergency response
- Strong communication and teamwork skills with residents, families, and clinicians
- Ability to triage changes in condition and escalate appropriately
- Familiarity with long-term care regulations and quality standards
