Roles & Responsibilities

• Perform comprehensive home health assessments and create individualized, outcomes-focused treatment plans
• Deliver high-quality therapeutic interventions to improve mobility, function, and independence
• Document patient encounters thoroughly and timely, Epic experience preferred but not required
• Conduct home safety evaluations and recommend adaptive equipment and environment modifications
• Educate patients and caregivers on exercises, fall prevention, and self-management strategies
• Coordinate care with nurses, physicians, and interdisciplinary team members to ensure continuity
• Maintain required credentials and complete Trinity specialty competency testing and checklists

Qualifications

• Current Physical Therapist license with ability to verify licensure (License/Certificate Verification required)
• BLS certification required
• Trinity specialty-specific competency testing and Trinity skills checklist completion required
• Epic experience preferred but not required
• Comfortable delivering care in patients' homes and adapting to varied home environments

Knowledge & Skills

• Strong knowledge of home health evaluation and intervention techniques
• Skilled in patient-centered care, mobility training, and discharge planning
• Proficient documentation skills; familiarity with Epic is a plus
• Excellent communication and teaching skills for patient and caregiver education
• Ability to manage time effectively, prioritize caseloads, and work independently in the field
