Roles & Responsibilities

• Provide high-quality, compassionate bedside care to residents as a licensed practical nurse
• Administer medications and treatments safely and accurately according to orders
• Perform and document thorough patient assessments and report changes to RN/MD
• Collaborate with the interdisciplinary team to develop and implement care plans
• Maintain accurate, timely charting in the electronic medical record
• Support admissions, discharges and transfers with professionalism and care
• Champion infection control and patient safety practices on every shift

Qualifications

• Current, unencumbered LPN license appropriate for practice in West Virginia
• Basic Life Support (BLS) certification required; additional certifications a plus
• Experience in long-term care or skilled nursing preferred but new grads encouraged to apply
• Ability to pass background check, drug screen and meet facility onboarding requirements
• Strong communication skills and a team-oriented attitude

Education

• Graduate of an accredited Practical Nursing (LPN/Vocational Nursing) program

Knowledge & Skills

• Solid medication administration and documentation skills
• Comfortable with resident assessment, wound care, and ADL support
• Familiarity with electronic medical records and charting best practices
• Good clinical judgment, time management and prioritization under busy conditions
• Excellent interpersonal skills for working with residents, families and staff
