Roles & Responsibilities

• Evaluate and provide skilled speech-language and swallowing therapy for residents in the SNF
• Develop and implement individualized treatment plans that improve communication and swallowing function
• Collaborate closely with nursing and the interdisciplinary therapy team to coordinate quality care
• Document assessments, goals, and progress in accordance with CRG policies and procedures
• Meet productivity expectations (therapist productivity requirement: 90%) while maintaining high-quality care
• Be onsite and engaged with residents — presence in the building is required to receive guaranteed hours

Qualifications

• 1-3 years of experience as an SLP (as specified)
• Experience working in a skilled nursing facility (SNF) or with geriatric populations preferred
• Able to work onsite consistently and follow CRG policies & procedures
• Comfortable meeting productivity expectations and workplace dress code (black scrubs required)

Knowledge & Skills

• Skilled in speech, language, cognitive-communication, and swallowing evaluation and treatment
• Familiarity with SNF documentation, regulatory expectations, and productivity tracking
• Strong interdisciplinary communication and collaboration skills
• Professional presentation and adherence to facility dress code and policies
