Roles & Responsibilities

Perform thorough speech-language evaluations to assess resident needs
Provide daily treatment sessions focused on communication and swallowing function
Plan and implement individualized treatment plans and adjust as residents progress
Manage and coordinate safe, effective discharges with the care team
Document treatment, progress, and discharge summaries accurately and timely
Collaborate with interdisciplinary team members to optimize resident outcomes
Follow Continuum Rehabilitation Group policies and maintain required productivity (90%)

Qualifications

Primary skill: SLP (Speech-Language Pathologist) — must be able to practice per facility rules
1-3 years of experience (as listed in the order)
Prior SNF experience strongly preferred
Ability to follow CRG policies and productivity expectations
Willingness to be on-site and follow facility dress code (black scrubs)

Skilled in conducting speech-language evaluations and developing treatment plans
Experience delivering daily treatment sessions in a skilled nursing setting
Familiarity with discharge planning and care coordination in SNF
Strong documentation practices and productivity management
