Roles & Responsibilities

• Conduct comprehensive speech-language evaluations for SNF residents
• Provide daily, hands-on therapy sessions tailored to each patient
• Plan, implement, and adjust individualized treatment plans
• Manage and coordinate discharges to ensure smooth patient transitions
• Document progress and maintain accurate clinical records
• Collaborate with the therapy team and facility staff to deliver high-quality care
• Meet productivity expectations and contribute to a results-oriented environment
• Follow CRG policies and procedures and adhere to facility dress code (black scrubs)

Qualifications

• 1-3 years of relevant experience (as stated)
• Prior Skilled Nursing Facility (SNF) experience strongly preferred
• Willingness to follow CRG policies and meet productivity expectations
• Able to be present onsite to receive guaranteed hours and work within facility guidelines

Knowledge & Skills

• Skilled in SLP evaluations, daily treatment, and discharge planning
• Experience working in a skilled nursing facility environment
• Strong documentation and time-management skills
• Team-oriented communication and collaboration with interdisciplinary staff
• Ability to meet productivity targets and follow organizational policies
