Location: Carmichael, California

Start Date: 07-07-2026

End Date: 10-06-2026

Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, with rotating weekends (at least one full weekend per month)

Hours per Week: 40

Candidate Pay Rate: $82 per hour

Roles & Responsibilities

Coordinate comprehensive care for patients across all hospital units
Complete initial assessments, Interqual (IQ) reviews, and discharge planning
Manage utilization review (UR) and discharge planning for a caseload of 15-20 patients per day
Collaborate with multidisciplinary teams to ensure effective care transitions
Communicate with payers, uninsured patients, and homeless populations to facilitate care
Utilize Cerner and Interqual systems for documentation and criteria adherence

Qualifications

Prior recent CA Dignity experience required (candidates without it will not be considered)
BLS certification required
Experience with Interqual criteria and Utilization Review/Discharge Planning
Proficiency in Cerner EMR
Strong collaboration, teamwork, and communication skills
Ability to reprioritize throughout the day in a fast-paced environment

Knowledge & Skills

Knowledge of case management principles and discharge planning
Familiarity with Interqual and Cerner systems
Understanding of payer types, including uninsured and homeless populations
Effective communication and interpersonal skills
Ability to work both independently and as part of a team
