Contract Type: Contract

Shift Type: 8-hour shifts, 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, rotating weekends and holidays

Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Develop and manage daily patient lists for the palliative care team
Review patient charts and assess needs to coordinate comprehensive care
Initiate goals of care discussions and educate patients and families on disease process
Facilitate advance directives and POLST completion
Conduct family meetings and provide end-of-life support
Participate in daily interdisciplinary rounds to optimize patient care

Qualifications

2-5 years of recent experience in palliative and/or hospice settings
Acute care experience required
ICU or oncology experience preferred
Current BLS certification required; ELNEC certification preferred
Strong clinical thinking, communication, and organizational skills
A passion for serving patients, families, and team members
