Roles & Responsibilities

• Conduct timely post-hospital and ED follow-up phone calls (including 48-hour TCM calls) to ensure safe transitions
• Apply case management principles to acute care patients to support appropriate utilization and level of care
• Facilitate discharge planning and coordinate referrals to skilled nursing, home health, outpatient providers, and infusion therapy
• Identify barriers to patient flow and collaborate with interdisciplinary teams to resolve them
• Recognize patients who need Case Management or Social Service referrals and initiate appropriate interventions
• Complete accurate, timely documentation in Epic and Cerner to support continuity of care
• Serve as a resource to the ED Hospital Liaison and support effective communication across teams
• Participate in process improvement efforts to reduce unnecessary admissions, lower readmission rates, and improve flow metrics
• Maintain patient confidentiality and HIPAA compliance at all times

Qualifications

• Active, unencumbered Registered Nurse (RN) license in Rhode Island (required)
• Minimum 5 years of related clinical experience in Acute Care or Case Management
• BLS certification (required)
• Med/Surg RN background strongly preferred
• Knowledge of utilization review, discharge planning, and third-party payor regulations
• Strong interpersonal, oral, and written communication skills
• Demonstrated analytical ability, leadership, and effectiveness working in teams

Education

• Bachelor’s Degree in Nursing (BSN) preferred; equivalent experience accepted

Knowledge & Skills

• Telephonic case management and Transitions of Care expertise
• Documentation proficiency in Epic and Cerner
• Utilization review and discharge planning processes
• Familiarity with third-party payor requirements and NCQA timeliness benchmarks
• HIPAA compliance and patient confidentiality best practices
• Excellent communication, care coordination, and documentation accuracy
• Analytical thinking and ability to support process improvement initiatives
