Candidate Pay Rate: $49 per hour

Roles & Responsibilities

Deliver compassionate physical therapy care to patients across greater Sacramento county in their homes
Treat a diverse caseload including cardio, pulmonary, orthopedic, neurological, and geriatric patients
Help patients regain strength, mobility, and independence with personalized treatment plans
Collaborate with interdisciplinary teams to ensure the best outcomes for every patient
Document visits thoroughly and maintain accurate mileage records for reimbursement

Qualifications

Active Physical Therapy Assistant license in California
Prior home health experience preferred
New graduates considered only in exceptional cases
Strong communication and time-management skills
Ability to work independently in various home settings

Knowledge & Skills

Experience with a variety of diagnoses including cardio, pulmonary, orthopedic, neurological, and geriatric conditions
Comfortable managing a 5-6 patient daily caseload
Skilled at building rapport with patients and families
Detail-oriented with excellent documentation habits
