Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes throughout Yolo County
Assess and evaluate each patient's therapeutic needs while developing comprehensive care plans
Guide patients and families through rehabilitation exercises and therapeutic appliance use
Monitor progress and communicate changes to physicians to optimize recovery outcomes
Collaborate with home health aides and caregivers to ensure seamless care coordination
Document clinical progress and celebrate patient achievements toward their goals

Qualifications

Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Valid driver's license and reliable transportation for home visits
Two supervisory references from Physical Therapy modality (one within 12 months, one within 3 years)

Knowledge & Skills

Home health physical therapy assessment and treatment techniques
Patient and family education for therapeutic exercises and equipment
Understanding of Medicare and Medi-Cal regulations
Excellent documentation and clinical reporting abilities
Compassionate approach to caring for patients in their home environments
