Roles & Responsibilities

Evaluate and treat patients with a variety of conditions in a skilled nursing setting
Develop and implement individualized treatment plans to improve mobility and function
Document patient progress and communicate with interdisciplinary care team
Educate patients and families on exercise programs and home safety
Supervise and delegate tasks to physical therapist assistants and aides

Qualifications

Active California Physical Therapist license required
Doctorate in Physical Therapy (DPT) from an accredited program
Current BLS certification
Experience in skilled nursing or geriatric rehab preferred
Excellent communication and interpersonal skills

Knowledge & Skills

Knowledge of geriatric rehabilitation and common conditions
Proficiency in electronic medical records systems
Strong clinical assessment and treatment planning skills
Ability to work independently and as part of a team
Time management and organizational skills
