Roles & Responsibilities

Provide exceptional physical therapy services to residents in a skilled nursing facility
Develop and implement individualized treatment plans to improve mobility and function
Collaborate with interdisciplinary teams to ensure comprehensive patient care
Float between sister locations in Perrysburg and Monclova as needed
Document patient progress accurately and timely
Educate patients and families on therapeutic exercises and home programs

Qualifications

Active Physical Therapist (PT) license in Ohio
Doctorate in Physical Therapy (DPT) preferred
New graduates are welcome to apply
Experience in SNF setting is a plus, but not required
Ability to float between two nearby facilities
