Roles & Responsibilities

• Assess patients’ functional mobility, strength, balance and pain to create individualized treatment plans
• Provide skilled therapeutic interventions to improve mobility, independence and quality of life
• Document patient progress clearly and timely in the clinical record
• Collaborate with nursing and rehab teams to coordinate holistic care plans
• Educate patients and caregivers on safe transfers, home exercise programs and fall prevention
• Monitor outcomes and adjust treatment plans to achieve measurable functional gains
• Support discharge planning and provide clear recommendations for ongoing care

Qualifications

• Current, unrestricted Physical Therapist license in Texas (or ability to obtain prior to start)
• Experience in skilled nursing, long-term care, or inpatient rehab preferred
• Current BLS/CPR certification (or willing to obtain)
• Strong clinical assessment and manual therapy skills, with solid documentation practices
• Excellent communication skills and the ability to work well within an interdisciplinary team

Education

• Graduate of an accredited Physical Therapy program (DPT preferred)

Knowledge & Skills

• Knowledge of therapeutic exercise, gait training, balance and fall prevention strategies
• Skilled in patient assessment, treatment planning and outcome measurement
• Familiarity with skilled nursing facility regulations and documentation expectations
• Comfortable with mobility equipment, adaptive devices and safe transfer techniques
• Strong patient education and motivational coaching abilities
