Roles & Responsibilities

• Evaluate and treat residents in skilled nursing and post-acute settings to help them reach meaningful functional goals
• Develop and implement individualized treatment plans that prioritize safety, independence, and quality of life
• Collaborate closely with an interdisciplinary team to coordinate seamless, patient-centered care
• Document accurately and timely in accordance with facility and regulatory standards
• Participate in care conferences and drive ongoing quality improvement efforts
• Communicate clearly with residents and families to educate, motivate, and support rehabilitation progress

Qualifications

• Active California Physical Therapist license (required)
• Strong clinical skills in evaluation and treatment planning
• Excellent communication and teamwork abilities
• Skilled nursing or post-acute care experience preferred
• Dependable, flexible, and committed to high-quality, compassionate care
